Case Study 1

A recent assignment by AAI involved audit of a major airline facility for operational availability, occupational health & safety and legal compliance. The plant assessed supplied building services. The review was ordered when an extended outage jeopardised business continuity.

The review found that faults in equipment and software caused regular downtime. Due to reduced manning and poor service response time, the client was planning to automate even more of the plant function. AAI identified problems in their original solution that could have resulted in a major fire or explosion. Other alternatives involving reduced risk were suggested.

Several compliance matters were also identified, which the business in question viewed as valuable.

 

Case Study 2

A consulting task, performed by AAI for an importer of equipment, delineated major duties and liabilities that they could be exposed to under proposed legislation. Such legislation, despite appearances, is often complex and convoluted. This is sometimes due to a lack of detail in the statutes that can only be found in referenced documents such as standards.

 

Case Study 3 - A Cautionary Tale

Another recent case study does not involve our clients, but makes interesting reading. A major incident involved the rupture of a pressure vessel supplying building services. There was no injury and property damage in the immediate vicinity was relatively small. However, the secondary damage, business interruption and knock on effects generated a reported cost in excess of One Hundred Million Dollars.

Alleged causes of the rupture include inadequacies of design and/or manufacture of the pressure vessel. This incident provides a warning that procurement of hazardous equipment, especially for major facilities, should not be left to chance.

 

Case Study 4

Recent accidents at utilities, public sector and defence sites have pointed to a lack of third party supervision in engineering contracts. This is AAI's area of expertise.
